A chronic condition is a health condition or disease that is long-term and affects a person’s quality of life over time. Such illnesses included heart disease, cancer, stroke, lung diseases such as asthma and Chronic Obstructive Pulmonary Disease, and diabetes. Many chronic diseases are caused by a shortlist of risk behaviors including unhealthy eating, inactive living, and tobacco use.

The Healthy St. Mary’s Partnership invites all who live, work, or play in St. Mary’s to complete the Community Health Assessment. The purpose of this survey is to get opinions about community health problems in the county.
HSMP will utilize the results of the survey, along with other available information, to identify the most pressing issues that can be addressed through community action. Input through the survey is anonymous. The survey will take 5 to 10 minutes and is available in English and Spanish. The survey is open until June 30, 2026.
